Privacy Policy
In accordance with the National Privacy Provisions 2001.
General
In filling out the practice’s forms, and during your consultations, various personal, administrative, and medical details concerning you will be recorded and kept in your medical file. Also in your file will be reports supplied by other practitioners such as xray, pathology etc. These have always been, and will continue to be, kept strictly confidential. The staff understand and are bound by the strictest of confidentiality requirements when handling your records.
Ordinarily information is not released without your consent. However, as has been traditional, in order to best care for you, reports regarding your condition, its management, and progress will be routinely sent to your referring doctor and other medical and paramedical people named by you as involved in your care, e.g. other doctors, therapists, rehabilitation providers, medical indemnity organizations, medical insurance companies, etc. as appropriate. Occasionally the law may require this practice to release information without your consent, e.g. by subpoena.
If a third party e.g. Workers’ Comp or Third Party insurer is paying for your care
i) copies of reports will be sent to the relevant insurer or third party payer, and/or their agents, for example rehabilitation providers, human resource managers, etc.
ii) in these cases you are entitled to obtain copies of your records from the insurer under the Workers’ Compensation Act
iii) additional reports may be requested and paid for by third parties to be used in legal or other proceedings, these must be accompanied by written permission from you before they are compiled and released.
You have the right to access your medical records. Should you wish to do so please ask for our handout “Accessing Your Medical File” which will explain the safeguards, procedures, and fees involved.
